Generating an Assessment Report
Once you've completed and saved an assessment, Metanoa can turn the form answers into a structured clinical report for you. This saves significant time compared to writing up findings manually and gives you a strong starting point you can edit before signing.
What the Report Contains
The AI reads through all the questions and answers in the assessment and produces a written clinical narrative. The content comes entirely from what was entered in the form, so the report reflects the actual assessment data rather than anything invented.
Reports generated from assessments are suitable for clinical records, IEP documentation, referrals, and family updates depending on how you edit and use them.
Where the Generate Button Appears
The Generate Report button only appears when you're viewing an existing saved assessment submission. It won't appear on a brand new form you've just opened because there's nothing saved yet to generate from.
If you don't see the button, save the form first, then reopen it from the Completed Assessments table.
How to Generate a Report
Let's Generate an Assessment Report
- Save the assessment if you haven't already, by clicking
Savein the form - Close the form and find it in the Completed Assessments table
- Click the assessment row to reopen it
- Find the Report section at the top of the assessment modal, above the form
- Click "Generate Report" and wait while the AI processes the form data
- Read through the report carefully and edit anything that needs adjusting
- Save the assessment to store the report alongside the form answers
And you're all set! The report is saved with the assessment and ready to print or share.
Getting a Good Report
The quality of the report depends directly on how thoroughly the form was filled in. Forms with detailed, specific answers produce detailed, useful reports. Forms with very short or vague answers will produce thinner output.
The AI needs at least a meaningful amount of completed fields to generate a report. If the form is mostly empty or answers are placeholder text, you'll see a message saying there isn't enough data to generate from. The fix is to go back into the form, add more complete answers, save, and try generating again.
Editing the Report
After generating, the report appears in an editor where you can change anything you like. Add clinical context the form didn't capture, adjust the language, remove sections that don't apply, or expand on areas that need more detail. The AI gives you a strong first draft, and your professional judgment shapes the final version.
The report isn't saved until you click Save on the assessment, so always save after editing.
Signing Locks the Report Too
Once you sign the assessment, the report is locked along with the form answers. Make sure the report is exactly how you want it before signing. See Signing and Printing for the full signing workflow.
