Customising the Client Form
Every organization works differently. A pediatric therapy center needs different information about a client than an early intervention program or a school-based service. Metanoa is built to handle this by giving every organization a fixed set of fields that everyone uses, plus a fully customisable form that your Admin can shape to fit exactly how your team works.
The Fixed Fields
When you add a new client, you'll always see the same core fields regardless of which organization you're in. These are name, profile photo, date of birth, gender, department, phone, and email.
These fields are standard across Metanoa because every organization needs them. They can't be removed or rearranged, and they work the same way everywhere.
The Customisable Part
Below the fixed fields, you may see additional fields that are specific to your organization. These come from a customisable form your Admin has built in the Library, and they appear as a natural extension of the Add Client form so the whole thing feels like one continuous experience.
What those fields look like depends entirely on what your organization needs. Here are some examples of how different organizations use this.
A pediatric therapy center might add fields for the client's school name, year group, and the name of their class teacher, so therapists always have that context when coordinating with schools.
An early intervention program might capture funding type, referral source, and the referring professional's contact details, since that information is needed for reporting and billing.
A community-based service might add fields for a secondary caregiver's name and contact number, or a preferred language for communication, so all staff know how to engage with the family from day one.
A clinic with multiple service streams might use the form to capture which program a client is enrolled in, so records can be filtered and reported on by program type.
Where These Fields Appear
The customisable fields appear in two places: on the Add Client form when creating a new client, and on the Edit Overview form when updating a client's details later from their Profile tab. So anything you can fill in at creation, you can also update at any time.
For Admins: Setting Up the Customisable Form
The customisable client form is managed in the Library under Core forms. Your Admin can add, edit, or remove fields there, and any changes appear on the Add Client and Edit Overview forms straight away for all staff.
