Signing and Printing

Signing finalises an assessment and creates a permanent clinical record. Printing lets you generate a PDF of either the filled form or the written report. Both are available from inside the assessment modal.

What Signing Means

When you sign an assessment, you are confirming that the information is accurate and complete. Signing locks the entire assessment, including all form answers and any report content, so that nothing can be changed afterwards.

Heads up, signing is permanent. There is no way to unlock or edit a signed assessment, so take the time to review everything carefully before you confirm.

How to Sign an Assessment

You can only sign an assessment that has been saved at least once. The signature option won't appear on a form you've just opened for the first time without saving.

Let's Sign an Assessment

  1. Open the completed assessment from the Completed Assessments table on the client's profile
  2. Review all the form answers and make any final edits while you still can
  3. Review the report if one has been generated, and edit it if needed
  4. Add your signature in the signature field
  5. Click Save and a confirmation dialog will appear asking you to confirm the signing
  6. Confirm and the assessment is signed and locked

The assessment now shows as Signed in the Completed Assessments table and no further edits are possible.

What Happens After Signing

Once signed, the assessment becomes a read-only record. You can still open it to view it, print it, and share it, but the content is fixed. It will show a Signed status in the assessments table so your team knows it's finalised.

If you spot an error after signing, you'll need to discuss the situation with your Admin as there is no self-serve way to unlock a signed assessment.

Printing an Assessment

There are two separate print options inside the assessment modal, one for the form and one for the report.

Print Form generates a PDF of the filled assessment form, showing all the questions and the answers given. Use this when you need a record of the raw assessment data.

Print Report generates a PDF of the written report, whether that was AI-generated or typed manually. Use this when you need a clinical narrative suitable for sharing with families, other professionals, or for inclusion in referral documents.

Both options are available regardless of whether the assessment has been signed or not.
